How to Apply for DDD Services
To apply for DDD Services:
- Complete the Full Application for Determination of Eligibility.
- Submit the application via:
- Email: Scan and send to DDD.NJApply@dhs.nj.gov.
- Mail: Send it to the Community Services Office in your county.
- In-person: Deliver it directly to your local Community Services Office.
Applicants must also complete the NJ Comprehensive Assessment Tool (NJ CAT), which determines service needs and funding tiers. The eligibility process can take up to 60 days.

What is DDD?
The Division of Developmental Disabilities (DDD) in New Jersey provides essential services for adults aged 21 and older with intellectual and developmental disabilities (I/DD). These services aim to enhance independence and quality of life through support coordination, Medicaid waiver programs, housing assistance, job training, and more. Eligibility requires proof of a developmental disability manifesting before age 22, Medicaid eligibility, and New Jersey residency.
